It is extremely important your entries be sent in on time.  Thank you!

Fill in the form and hit the submit button at the bottom of the form.  After you hit submit the form will go to my e-mail address and a confirmation page will be displayed.  Print the confirmation page for your records.  I will confirm receipt within 24 hours.

If you have more than 10 students Put the first 10 on this form, filling in all of the contact information. 

When you submit the forms with students 11-20 and 21-30,etc.

You do not need to fill in all the contact information again, just indicate the page number after the school name,  Slapout Page 2 in the School box.

Print each page before you hit submit.  There is a possibility you will receive an error message when you hit submit.  My confirmation pages are not working right now.  The forms are working and coming to my e-mail box, even though you receive the error message.  IF you don't receive a confirmation e-mail from me - fax the printed pages to 580-654-1852.
