We are changing scoring machines this year.  The new machine requires additional data for each student. 

It is extremely important your entries be sent in on time.  Thank you!

Fill in the form and hit the submit button at the bottom of the form.  After you hit submit the form will go to my e-mail address and a confirmation page will be displayed.  Print the confirmation page for your records.  I will confirm receipt within 24 hours.

 

If you have more than 10 students Put the first 10 on this form, filling in all of the contact information. 

When you submit the forms with students 11-20 and 21-30,etc.

You do not need to fill in all the contact information again, just indicate the page number after the school name,  Slapout Page 2
