Either print the information and fax to 580-654-1852 or fill in the form and submit online by clicking the submit button at the bottom of the form. Once you've submitted the form a confirmation page will display - print that page for your records.

$5.00 registration fee may be paid by purchase order, check or cash.
Registration fee may be paid in advance or on site.
A calculator is required to work the math and science problems. Bring your calculator.
